Internship ...

Hey as most of you know i started my internship on the first of august 09 in Riyadh Military Hospital ( RMH )

Startig Pediatrics ...

and i call myself the soloest .. cause i am the only one i know there in pediatrics :D

whatever in internship you face ... PEOPLE yeah that strange species ... so

new people = new experiance = new thoughts & ideas .. = new stuff for this blog

so i am thinking of starting a series of posts about internship ...

ACT I :

RMH - PEDIATRICS - FIRST ONCALL 12:01 AM

I was lucky i was oncall with a resedent :) told him it is my first on-call so he said really ?! i am going to spoil You faisal i am going to take the calls :D so i was a spectator most of the time .. So we were called on a patient With para-influnza virus .. i thought it was a waste to wear the infectouse control and get in if i am not adding anything

so my dear senior went inside ...

i waited by the door ... trying to listen if there is any history or something ...

a minute passed ... and then complains started

Doctor if we want to file a complain about the medical care here is this hospital where can we go ?

I am a Believer that you as a doctor should listen to your patient fully listening to all his/her complains ...i think it is part of the job ..

1 , 3 , 5 , 6 minutes ... and i am listening non-sense talk .. specially at 12 at night with an on-call Doctor ...

" it feels as if we are not in our country " " i was in hafer al bateen ( rural city ) and the hospital there was better " <<>

then i did a weird thing ... i just went to the phone ... picked it up .. and paged my senior ... trying to give him a window to exit that damn room ( after all there was an addmission in the ER so we had our Hands full )

when my seniour came out he said " i don't want to get into that room again :| " throught the chat .. he said she was Trying to Show that she is Educated and read everything about her girls syndrome ( Down syndrome )

The night goes on ... and we went back to the damn room ... this time i dressed up and entered as the doctor asked me if i want to try and draw blood . hmmm when i dressed and entered he started draing it :| and i was standing there infront of the patient .. my only explination was that when he saw the mother he remmbered the headach so i was just calming the baby down .. and then i noticed a lot of scars on her chest and abdomen .. small circular scars at first i said aaah most likely varicella scars ( chicken pox) ... but hey .. they are so damn lined up in columns .. :| :| :| I asked the doctor ... he said cuttary.. ( Traditional treatment ) :| i counted over 15 .. in a 3 years old Down syndrome girl ... he said .. and she was trying to show that she is educated .....

ACT II

I am calling this Act as Redefining medicine

Same night , Same on-call but earlier 10 PM ..

VIP section .. patient want to go out on pass .. come and sign already discussed with the consultant

:| ... :| ...

a 9 year old kiddo .. also Influnza Virus ( not H1N1 By the way ) ..

we got into the room , looked at his druh chart ..

confirmed viral

and he was almost on three ( can't remmber exactly) antibiotics ... :| ...

i just Thought aaaah this redefines medicine ....

Act III

TODAY 8-8-09 in The ward Pt is discharged and was given an apointment after two weeks ( closest possiable )

my Senior went out got caught with the dad ... why is it after two weeks ?! the doctor said it is gonna be after one week ...

oh my god .. i asked the nurs this is the closest possable doctor ..

i told him okik sir i will call my senior to confirm if it would matter a week of two or not ?!

called >> didn't answer

i told him okik you can go and if anything happened you can come to the ER at anytime

he said no , our medication would have finish by then , and ER is a mess you can't get care through there ( although their addmission was through this ER ... )

he said i want it after a week ...

i told him Sir i am new here and i don't know the system yet the thing i can do for you is that i will talk to my senior and answer you ...

Called >> no answer

he is not answering ....

he said OK Give me his number ... ( loved myself here ) i just Said .. NO i actually loved that i said it without thinking ... aaah i just love my intuition ..

he wraped the thing politly ( Strange .. i was getting angrier by the second ) i told him okik ER if she needs anything .. he went back to the same dilemma ..

i just said .. sir thats what we can give .. and this is what this hospital can offer what i can do now to be in your service is .. i will talk to my senior tommrrow morning and i will call you if we could find a sooner apointment ...

Yeah being a patient is well respected but sometimes you must Say .. NO .. just plain simple and Straight forward ..

ACT IV

today same ward earlier ..

doctor is that a picture to be seen in a hospital .. ( a toilet with some of the pain worn off ) actually when i saw it in the mobile .. i was woundring what wrong in it ? ...

i asked ... was there water in that bathroom ?!

he was amazed and said why ?!

i told him no really was there any ?! he said Yes

i answered .. okik good you must see RCH ... then

come on Quality improving is a great concept .. when it is about something matters ...Don't give me a headach on some paint :|
